Abstract

|
목적: 본 연구는 국내 상급종합병원과 여성전문병원의 산부인과 병동간호사에 대한 산후 간호중재 기록지 개발 및 산후 간호중재 개입을 평가하기 위해 수행되었다.
방법: 이 횡단적 조사 연구는 Time-Motion(TM) 방법을 통해 탐색하였다. 24시간 동안 4개 병원 12명의 간호사를 외부관찰 하여 자료수집 하였다. Mann-Whitney U test와 independent t-test는 직접 / 간접간호 활동의 빈도와 제공시간 분석에 사용되었다. 두 그룹 간 빈도 차이는 x2(Fisher 's 정확 검정) 검정을 이용하여 측정하였다. IBM SPSS 22.0 통계 프로그램을 이용하였으며, 통계적 유의수준 ?=0.05로 설정하여 산출하였다.
결과: KPCS-1 (한국형 환자분류도구-1)을 기준으로 여성전문병원은 3군과 상급종합병원은 4군 범위로 나타났다. 직접간호의 시간차에는 상급종합병원은 791분, 여성전문병원은 399분으로 392분의 차이를 보였다. 간접간호의 시간차는 여성전문병원이 2,415분 상급종합병원은 2,080분으로 335분 차이를 보였다. 두 기관 간 간호업무량의 전체 평균에서는 차이가 없었다. 개인시간에서도 차이가 없었다(p>.05).
결론: 상급종합병원에서는 고위험산모 간호가 강점이었고, 여성전문병원에서는 모유수유 간호가 강점으로 병원 간 서로 벤치마킹이 필요하다

Purpose: This study was done to assess development and postnatal care interventions in postnatal care intervention records for maternity ward nurses in tertiary hospitals and women’s hospitals in South Korea.
Methods: This mixed-method research was a Time-Motion (TM) study. Data were collected through external observation of 12 nurses in 4 wards over 24 hours. Mann-Whitney U test and independent t-test were employed for the analysis of frequency and provision time of direct/indirect care activity. x2 (Fisher's exact test) was utilized to determine the difference in frequency between two groups. IBM SPSS 22.0 statistical program was employed for calculation. All statistical significance levels were at ?= .05.
Results: According to the KPCS-1 (Korean Patient Classification System-1), women’s hospitals are group 3 and tertiary hospitals, group 4. With respect to time difference in direct care, tertiary hospitals showed 791 minutes and women’s hospitals, 399 a difference of 392 minutes. For time difference in indirect care, women’s hospitals had 2,415 minutes while tertiary hospitals, 2,080, a difference of 335 minutes for women’s hospitals. No difference was found in the average total care workload between the two institutions. Individual time also showed no difference (p>.05).
Conclusion: High-risk maternal care strength in tertiary hospitals and breast-feeding strength in women’s hospitals need to be benchmarked with each other.
